How do I print off a physical to take to the doctor?

First, you need to begin a registration at your athlete's school. Go to aktivate.com

1. Click Login to Register
2. Open on the Parent Portal by clicking Click here to start/complete athlete registrations
3. Click on Start/Complete Registrations. This is on the left-hand side of the screen under the section "what would you like to do?"
4. Click View/Complete Registration
5. Click Physical Documents 
   1. NOTE: If you don't see physical documents, then you may not be far enough in the registration process. Continue to follow the directions given in the registration until you see Physical Documents.
6. Select the Physical examination requirement's name.
   1. NOTE: All schools label their documents differently so the wording on what this requirement is may vary
7. Click Download
8. The document will appear and you can select the print or download icon in the top right.
