Why Can't I see a Varsity Sport Eligibility Form?

If you cannot see a Varsity Sport Eligibility Form it is most likely because the name that is listed for the AD or Principal is incorrect. We recommend reaching out to the coach who submitted the form to ensure that the correct name was inputted.


A coach can find the name that the form was submitted to by following the steps below:

  1. Log into your Aktivate Account
  2. Click into the UIL Portal
  3. Click Navigation
  4. Select Eligibility Forms
  5. Click Varsity Sport Eligibility
  6. Click View next to the form you submitted
  7. Confirm that the AD or Principal's name is correct 
  8. If the name is not correct, click Update AD or Principal Info
